Medical weight loss refers to the proactive management of diet, fitness, and healthy behaviors conducted by a physician. These programs often include comprehensive metabolic testing and medical diagnostics to monitor physiological shifts during the course of a program.

While medical weight-loss programs have a much stronger success rate than most self-managed diet regiments, both are often focused on the same essential goal: improving an individual’s nutritional and exercise habits. Dr T. utilizes multiple medical techniques to custom tailor your weight loss program to your unique physiology and needs. You can do it and we will help you every step of the way.

If you have questions and want a free consult with the nurse Dori Josimovich 269-767-6510  (Mon-Thurs 10-6) to set up a free consult. Understand you will not be able to start a program until you have a phone consult with Dr. Topouzian.

If you know you want to start a program, and maybe have some questions, you can get your phone consult with Dr. T done and then make your appointment with Dori.

To get your phone consult follow these directions:

To fill out your questionnaire for Dr. T you can do it 2 ways.

1. Get the male or female questionnaire at the bottom of this paragraph. Click on the form you need, download and save the form.
2. If you want to fill it out by hand, print the form. Once filled out you can fax the form to the number at the top of the form or you can scan the form and email it as an attachment to the email at the top of the form.
3. If you want to fill out the form on your computer, you need Adobe reader. You can get it for free at adobe.com. Once you have downloaded adobe, open the Adobe program and choose Fill and Sign, then choose Select a File and open the form you just downloaded.
4. Once you have the form filled out, choose File and Save as, name your file.
5. Now open your email, attach your file and email the form to Dr. T at drthelpdesk@gmail.com in subject put “Phone consult for Weight Loss”.
6. Then call Dr. T’s office at 248-302-0473 to schedule your phone consult with the doctor.
7. Once approved call Dori to schedule your first appointment. 269-767-6510
8. You MUST bring a copy of this form with you on your first appointment with Dori.
